· Actively engage with clients to promote wellness and preventive care. This involves educating them on the importance of regular check-ups, screenings,vaccinations, and maintaining a healthy lifestyle.
· Collaborate with healthcare providers to develop personalized care plans that prioritize wellness and prevention for each client.
· Conduct health assessments and screenings to identify potential health risks and recommend preventive measures.
· Coordinate screenings for conditions such as hypertension, diabetes, cancer, or infectious diseases based on client or population needs.
· Provide resources and information on wellness-related topics, wellness program, and services.
· Coordinate referrals to specialists, preventive services, based on client needs.
· Ensure seamless communication between healthcare providers to optimize client care and follow-up.
· Serve as acentral point of contact between clients, healthcare providers, and other healthcare professionals.
· Facilitate effective communication, ensure proper documentation, and support smooth transitions of care.
· Regularlymonitor client progress in following preventive care plans, including adherence to recommended screenings and interventions.
· Follow upwith clients to address any barriers, provide support, and reinforce the importance of preventive care.
· Build relationships with community partners to expand access to preventive care resources and programs.
· Maintain accurate and comprehensive records of client interactions, care plans, and outcomes.
· Prepare reports and documentation as required by healthcare regulations and to gauge the effectiveness of wellness and preventative services.
· Be flexible to work between different sites.
· Perform other duties as assigned to support Wellness Aim & Objective.
· Possess a bachelor's degree in a healthcare-related field such as nursing, public health,health education, or healthcare administration.
· Posses valid registration and classification from the Saudi Commission for Health Specialties.
· Additional-certifications or training in care coordination, client engagement, or wellness programs would be advantageous.
· Proven work experience of at least three years as a clinical coordinator or similar role in a healthcare organization.
· Skills:
· Have strong understanding of the principles and practices of preventative care,including screenings, vaccinations, lifestyle modifications, and health promotion.
· Stay updated on current guidelines and best practices related to wellness and preventative services.
· Demonstrate excellent communication and interpersonal skills to engage with clients,healthcare providers, and other stakeholders.
· Be able to explain complex medical concepts in a clear and concise manner and build rapport with a diverse range of clients.
· Possess experience in care planning and coordination, including assessing client needs,developing centralized care plans, and coordinating services across multiple healthcare providers.
· Understand the importance of continuity ofcare and have the ability to navigate through healthcare systems.
· Have the ability to motivate and educate clients on the importance of preventive care.
· Exhibit strong organizational and time-management skills to effectively prioritize tasks, manage caseloads, and ensure timely follow-up with clients andhealthcare providers.
· Have the ability to handle multiple responsibilities and work within established protocols and guidelines.
· Possess the capacity to gather, analyze, and interpret data related to wellness and preventative services.
· Use data to identify trends, evaluate program effectiveness, and make informed decisions to enhance care delivery and outcomes.
· Be culturally sensitive and aware of the diverse backgrounds and healthcare needs of clients.
· Understand the impact of cultural factors on healthcare decision-making and be able toprovide equitable and client-centered care.
· Be proficientin using electronic health records, care coordination software, and other technology platforms commonly used in healthcare settings.
· Have good understanding of health information privacy.
· Work well within interdisciplinary teams, collaborating effectively with healthcare providers, community organizations, and other stakeholders to coordinate care and promote wellness.
· Possess strong teamwork skills to foster a culture of cooperation and shared goals.
